Please print these forms, fill them out as completely as possible and fax them to (530)-342-6506 at least 24 hours prior to your appointment, to allow time for the data to be entered. Bring the originals with you when you come in. Completing these forms prior to your appointment will shorten the wait time for you to see your care provider.

Make sure you have Adobe Acrobat Reader installed on your computer. You will need this to be able to view and print the forms. This software is free to use for an unlimited amount of time. Click on the hyperlink above for the form you want to print. Click on the “Open” button. When the form opens, click on the “Print” icon just above the form. Print the form to your home or office printer.
